An original health certificate in English, done within 14 days of arrival in Hawaii including rabies vaccine name, lot or serial number, booster interval, vaccination date and expiration date is required.

Dogs entering Illinois must be accompanied by a Certificate of Veterinary Inspection (CVI) issues within 30 days of enter showing the age, sex, breed and description of each animal and provide a complete destination name and address within Illinois.

These regulations do not apply to dogs consigned to hospitals, pharmaceutical companies, or licensed research institutes for research or teaching.

All dogs 16 weeks of age and older shall be vaccinated against rabies and the date of vaccination and manufacturer information must be included on the CVI.

The animals in the shipment must be free from any visible evidence of disease and cannot originate from an area under rabies quarantine.

If an animal is being transported to an Animal Welfare Licensee in Illinois, the CVI must also include a permit number issued by the Department.

Indiana

The certificate of veterinary inspection (CVI) must contain a physical description, such as breed, sex, age, and color that sufficiently identifies the individual animal. The CVI must be completed by a licensed and accredited veterinarian within the thirty (30) days immediately prior to the animal entering Indiana.

Every dog or cat that is 3 months of age or older must be vaccinated for rabies within the 12 months or within 36 months immediately prior to the animal entering Indiana depending on the vaccine used. Additional vaccinations are required for exhibition of dogs and cats in Indiana.

For the purpose of sale, a person may not import to or export from Indiana any dog under the age of 8 weeks unless the dog is transported with its dam.

A certificate of veterinary inspection is required for all dogs, cats and ferrets entering the state for longer than 30 days. The North Dakota State Board of Animal Health does not accept USDA APHIS Form 7001 for interstate movement of animals.

It shall be unlawful to bring a dog into this Commonwealth without a certificate of health prepared by a licensed graduate veterinarian, which certificate shall accompany the dog while in this Commonwealth.

A dog over 3 months and under 1 year of age shall have been vaccinated against rabies with an approved rabies vaccine. A dog over 1 year of age shall have been vaccinated or revaccinated against rabies after 1 year of age.

The certificate shall state that the dogs show no signs or symptoms of infectious or communicable disease; did not originate within an area under quarantine for rabies; and, as ascertained by reasonable investigation, have not been exposed to rabies within 100 days of importation.

Identification and Certificate of Veterinary Inspection are required for the importation of all dogs and cats that are imported into the State of Rhode Island.

Rabies: All dogs and cats three (3) months of age or older imported into Rhode Island must be accompanied by proof of rabies vaccination.

Within the 10 days prior to its date of entry into Virginia, a pet animal must be deemed healthy and free of infectious diseases after examination by an accredited veterinarian. Proof of examination must be submitted with the permit request and on a Certificate of Veterinary Inspection in a format approved by the State Veterinarian.

A pet animal brought into Virginia by a resident of Virginia or by a resident of another state who intends to make his residence in Virginia shall not be subject to the requirements of this chapter unless the pet animal is brought into Virginia to be offered for public adoption, transfer, sale, trade, or promotional incentive.
